All right,

It almost made me want to opt in, looks pretty good.

Eye candy, as in it is appealing and makes me want to see the video.

Couple tips, lose the scroll bar, make the whole presentation available with out having to scroll up or down.
Put it all on one page.

Other than that it looks pretty good, start marketing it!

I made the changes to the site you suggested. Take a quick look.

I am still adding the Privacy links ect. at the bottom.

Curiosity
I want to see what the video is about.
I love capture pages and marketing systems, the better the graphics or LCP I am more likely to opt in just to see what they are selling/marketing.

I think it looks great... Of course, you have to get it in front of the right people...

They only thing I would ad is a box that pops up when you click on the video to try and play it, that says "Hold on There, to see this video you have to put your info in the form to the right"...
Or something like that anyway

Hey Jamual ...
Have you tested the page yet?
If not, then consider this is just speculation until you get a LIVE audience in front of it that give you actual performance stats. With that in mind I would go live asap, then tweak.
But since you asked here's my $0.02 ...

Overall I like the page and am curious what's on the other side ... that's a good thing.

headline:
add some more leading or choose a font with more leading; the letters are too scrunched and makes the text look sloppy and possibly hard to read for some visitors.

sub headline:
use the same font as your headline, just not as bold or an alternate that is not as bold but looks the same. Too many font changes makes the page look cluttered and allows the eye to wonder too much.

text under video:
make it two lines instead of three and remove it from below the video.
consider using it as a headline and split testing the two instead.

video screen:
add the "play button" icon over the screenshot.
this has shown to increase conversions in numerous campaigns.
visitors that click and see the video doesn't play are more likely to optin because they already took action by clicking the play button. now they want it.

spam disclaimer:
Scrap everything except "we respect your privacy"
Talking about spam makes the reader think you are going to spam them, so don't even bring it up.

legal links:
You should add your legal pages and links to them ... Privacy, Earnings Disclaimer, Terms of Use

background:
something about the background is bothering me as a designer.
the blue block at the bottom of the page seems like an after thought and makes the page feel heavy.
just something to chew on. Maybe break it down to a smaller height and center your copyright and legal links.

the red arrow / instant access / call to action / form position:
the arrow is a little big and is pushing your form down the page
the "instant access" tag is doing the same thing and could be accentuated for a stringer call to action.

the text in the form should be cut down to the minimum ... tell them what to do in as few words as possible. The longer you make them read the more they have to think = lower conversions.

You can fit it all within the form border which will bring the video and form to a level position on the page = less wandering eye movement.

I would have to agree with Richard however if you are going to place a play button on the video image witch will certainly get the masses clicking on the static image, that you use a modal box, of which pops up when they click the image with a message saying fill out the form to gain accesss with form fields in this greybox pop up

the idea is while the are interacting with your site via the image click ----- that you take advantage of their psychological commitment to watch the video and embed a capture form in this popup
