Hi everyone,
I wish to create an online survey on my blog to get a feel about what my prospects wish to achieve. This will be an automated way for me to pre-qualify prospects & work or call those that take the effort of filling out the survey form.
As network marketers the best way to serve our prospects is to address their key problem areas & this is a way for me to get some leads to open up.
I'm looking to create a squeeze page with a short video, text description & a series of questions on it. I've seen people like Mia Davies do this just before launching her coaching programs.
I know there are services such as Web Survey Master & Survey Monkey but I need this one personalized with my blog URL.
If anyone out there know how to do this please help me out.

iContact has a survey tool. you can create surveys and link to it from your blog or newsletters or whatever.
its also free until you reach 500 subscribers.

Hi Stephens,
I saw your request rather late tonight. There are a couple things you need to do to create a custom squeeze page with a poll on your blog. The first is to use a theme or plugin that allows you to create custom pages (unless you know html and can create a custom page that way). The second thing you need is another plugin to build the poll or survey. These are both the free ways to do it.
If you are a Wordpress user, let me know and I'll suggest a couple plugins for you.

Use docs.google.com
it's the best tool. There are form templates there. you add your questions, create your poll, and when you're all done you take the Embed code to yourwordpress blog. use a Text Widget and paste the embed code.
or if you want, you can just paste the embed code into the SOURCE of any post (the design view wont cut it).
if you use TwentyEleven, you can remove sidebars on any post by changing the post template from the right.
What i love about Docs.Google.Com for this purpose is that every time someone fills the form, it automaticaly updates an excel spreadsheet for you with their responses.
also you can have it notify you every time the form is used. i use this on my site and am in love with the features.

You can still use SurveyMonkey or any other of these services.
You just need to include the page that surveymonkey create inside a page you create in your blog.
To do that, you can edit the page in wordpress, switch the editor to HTML and include the code for an iFrame. Some services give you the javascript snippet to put in your code. Others you need to create but it's easy, just look in google for html for an iframe.

Hi! I'm just new to the forums, and know that this is a few days old, but thought that I may be able to help you out a little as well.
SurveyMonkey is really good, and it is technically possible to embed a SurveyMonkey form on your website using iFrames. Check out this article that they posted on their website - http://help.surveymonkey.com/app/answers/detail/a_id/240/~/you-can-use-iframes-to-embed-the-survey-onto-a-page-of-your-website.
If you need a hand with the iFrame stuff, just let me know and I'd be happy to lend a hand. One question I have though, is what type of visitor you are wanting to fill out the form? Remember that it can be a deterrent to totally cold leads to ask them to fill out a giant survey or form. You might end up scaring some of them away instead of getting their contact information.
I normally suggest that you start your first form with a really basic Name, email, Phone form, then get in touch with them. Whenever you can, get on the phone, and then if you need to, follow-up with an email inviting them to your survey later on. I've found that moving the survey a little further down the sales cycle (essentially to when they're a 'warm' lead instead of just a visitor to your site) is the best way to go.
