JACOB’S HERITAGE FOUNDATION

Mission Statement: Jacob’s Heritage is a faith-based not for profit foundation founded in 2005. Our mission is to teach our youth through individual empowerment toward self-improvement in personal, social, academic, vocational and economic skills which would nurture a positive growth in spirituality, responsibility, education, and family structure.  

Purpose: To observe and strengthen the positive attributes in our youth while molding individual’s into leadership.

Executive Summary: While constructing our foundation with grass root concepts we will also structure the building blocks of our youth. We would be able to place young adults in grass root programs for self-improvement. Through youth development we plan to strengthen as many areas as needed in the lives of our youth so they will know, express and understand their full potential. Through mentoring young adults we believe we can initially demonstrate the nurturing it takes to develop our youth one soul at a time.

Plan of Action: Pick-up Drive is a scheduled transport pick-up for our youth. Our Pick-up Drive is a short term goal we plan to implement by June 2009. We structured this Pick-up- Drive only for the South Shore community. The South Shore community is our target area of operations and is located on Chicago's southeast side.  We are focused on implementing our Pick-up Drive for young adults age 14-21. This program allows the teen to go home after school and check in with their parents and when we receive a call from their parents to pick up the youth, we will be obligated to make the pick-up as soon as possible.  

There will be two drivers and two mini vans both transporting five teens to a location to implement mentoring according to our mission and purpose. At the start up of the Pick-up Drive we would like to conduct mentorship at remote locations, for example, libraries, YMCA's and community centers. One female director will mentor five female youth and the male director will mentor five male youth. We plan to achieve significant growth in membership during the remainder of 2009. 

Nevertheless, we are requesting donations for our transportation fund to purchase vans and would also request donations for our building fund for business operations. For these implementations we would like to meet our goal of $300,000 by June 2009.
